5. Signatures and $20 application fee will be signed and collected in person when you come for a tour/appointment.
(This application will not be processed until
we collect the $20 application fee)
6. Set a tour/appointment with admissions.

- Completed Application
- Copy of State or Government Issued ID
- Copy of Social Security Card or
Permanent Resident Card - Copy of High School Diploma, Official High School Transcripts, or GED Certificate (Please note - Home School Diplomas/Certificates are not accepted)
- $20 Non-Refundable Application Fee (Check made out to Aveda Institute Portland)
